Character Creation Form

This form will require you to provide the details of your proposed character. It is to be emailed to MASHBuPers@acalltoduty.com.  When sending the form back to the Personnel Manager, you must make sure it is in TEXT Format; otherwise it will not be accepted. This is done to speed up the processing of assigning your character to a M*A*S*H unit.
